Generally, 15 characters per line and a maximum of 5 lines is recommended (this may vary depending on the device).

- Names are written in Hepburn romanization (e.g. Saito → SAITO, Ota → OOTA)
- Date of birth: English month name, such as 21 JUN. 1973
- Blood type: A POS / O NEG (Rh type)
